Hi at your local branch,if you have one there is an advisor you will attachley be able to fill out the form for you ,it makes a lot of difference as they seam to know exactly what to do good luck.

You need to go online and research 1)parkinson's disease 2) go your local social security office and ask them what you need to do in order to qualify for social security disability benefits. Follow their instructions to the tee. Have patience it takes several months to get them to get the ball rolling GOOD LICK
I would highly recommend finding an attorney. I first tried to do mine on with only an advocate as it looked to me as I had a strong medical case. Fast tracked for DBS within a year of my Dx and still on high doses of meds, etc. I lost my case and appeals 4 years of fighting down the drain. Had to start a new case, I called the Michigan Parkinson Foundation for help. They were not happy that I was denied and recommended an attorney who deals with PD disability cases. He was not happy that I was denied either, but we had to start from scratch on a new case.
Check to see if there is a local resource/support center who might have an attorney they can recommend.
If he is a veteran talk to a veterans . Service officer,as well as social security.
