Hi - My Dad has Parkinsons. I attended his check-up and it seems like it would help my Mom and all of us if we were taking notes and tracking data about my Dad in between appointments. It would even help to have him do some of this on his own - water, exercise - physical and mind, other observations. Are such worksheets already available somewhere? Thanks.

Wow, that is a great idea. I don't have anyone to go to appointments with me (except my 10 year old daughter, and she isn't much help in that department) so I have been taking copious notes, but feel like I am not getting all of the info. Will look at the link for data sheets.
