Sinemet Vs Rytary

husband has been on Sinemet for 20 years and basically stopped working - going to start Rytary because of off time - does anyone have opinions on this?

well Rytary was terrible for him. It did nothing for the off time, he slept much more and moods were terrible to live with. It was like he was overdosed, even his therapist asked what happened! So back on Sinemet. but instead of 4 hours we are trying 3 hours - 25/100, 2 1/2 pills at 8 am, 11 am, 2 pm, 5 pm and 8. He is still wearing off but not as bad. Just have to admit there is no real cure for this disease and we are lucky he didn't progress sooner. Always hoping to help him more but its the biggest challenge ever and I am wore out. Moods and impulsiveness are horrible - impulse is running thinking he can go without walker and when he wakes from a nap he has to run for goodies to eat. He does get mean - got made cause "I" ate 2 of his cookies. Also, yes dementia is starting to come on stronger.

I was just watching something concerning 'Veterans and Parkinson's disease'. You mention something that a movement disorder specialist brought up. She stated 'People come to me, after taking a certain medication for many years and ask me why has the med stopped working'?. She chuckled and said-' It's not that the medication stopped working, but that your disease has progressed'. ..It's like the motion picture, when one gentleman says to another-'How far is it to town?..The second turn and says'-Don't know, I've never been there!!'

I love Rytary, it is so much better than Sinement. I have fever side effects and it works so much better. My insurance doesn’t have it on their formulary. They cover it after my neurologist writes a letter.

It’s the next logical step. Sinemet has diminishing returns over time and you don’t want to take to much, because that creates dyskinesia. Your Dr has made the logical choice of adding an off time medication.

There are no perfect solutions here, but this is the best choice at this time.

I have taken meds that worked for years and then stopped working I am always wary of trying new medications but rytary seems to be ok and does work tell him to expect some off days and I nausea or at least my experience but don’t stress about it I’m sure you know most of this already I use an apomorphine pump now because most medication wasnt working
