I live in Minnesota and we have 3 very good clinics that just treat PD and are mostly funded through endowments so no cost for many of the services. They have several neurologists who service the patients, provide, OT, PT, Speech, Counseling, Educational programs, exercise classes, yoga etc. Maybe check with your local PD Society to see if there are any such clinics in your area.
You probably need a movement disorder specialist, a neurologist who works with PD a lot. Try the National Parkinson's Foundation helpline (open Monday to Friday) - see for more info http://www.parkinson.org/find-help/helpline
I had physical therapy at home for a couple of months I learned a lot and try to use what they taught me both a regular physical therapist and an occupational therapist
I go to a movement disorder specialist in Rochester. She sent me to PT and it helped
I have the very same problems I believe that it is my PD. I see my neurologist in June
