When the symptoms interfere with your life. The meds only treat the symptoms, they don't cure anything. They don't slow down progression. (Exercise slows progression.) All meds have side effects. If you are able to live your life pretty normally, then wait to start. If a tremor or dystonia makes it hard to do your job, if the stiffness is painful, or if you are falling, then it is time to take meds.
