I had DBS almost 2 years ago. It has become fairly routine surgery. But they will remind you it is Brain surgery. For me, to be a candidate, I had to have had PD for 5 years, respond positively to carbadopa/levidopa, take a neuropsychological test, and have a 30% improvement in "off"/"on" test.

There are several good videos on youtube explaining DBS.
