Got my mobility and balance back with PT. Now three years later I have Orthostatic Hypotension which makes me lightheaded every day. I have a neurologist and cardiologist who are trying to help me through it. But no luck. I may get a good day once in awhile. It’s very debilitating. They say to move but I can’t with this lightheadedness. I get very depressed because I need to cancel out on a lot of events that I would enjoy. I used to always exercise but now I can’t do anything. Even walk. Help!

How can you manage orthostatic hypotension and lightheadedness that's preventing you from staying active after drug-induced Parkinsonism?

It's understandable to feel frustrated and depressed when symptoms persist despite stopping the medication. While most people with drug-induced parkinsonism recover within two months, some experience symptoms that last longer or progress differently.

The fact that you're working with both a neurologist and cardiologist is important. Since orthostatic hypotension can be a symptom that develops in Parkinson's disease, your medical team may need to reassess whether underlying Parkinson's was unmasked by the original medication.

Here are some approaches that might help:

Work closely with your doctors to:
- Consider whether imaging scans could clarify if this is progressing beyond drug-induced parkinsonism
- Explore medication adjustments specifically for orthostatic hypotension
- Discuss modified physical therapy that accommodates your lightheadedness

For managing daily life:
- Keep a detailed diary with dates, times, and severity of your lightheadedness episodes to share with your doctors
- Track which times of day you feel better, even slightly, and try to schedule activities then
- Consider seated exercises or very gentle movements while lying down as starting points

You're not alone in feeling this way. Depression is common when symptoms limit your activities, and it's worth discussing this with your healthcare team as well.
