Why does vascular parkinsonism often resist carbidopa/levodopa treatment?
Vascular parkinsonism is caused by small strokes in areas of the brain that control movement. Unlike Parkinson's disease, which involves the loss of dopamine-producing cells, vascular parkinsonism results from damaged blood vessels — so boosting dopamine levels doesn't always fix the underlying problem.
Only about 30% of people with vascular parkinsonism respond positively to levodopa. That said, doctors will often still try a trial of levodopa to see if it helps with slowness of movement or muscle stiffness. When medication doesn't help much, treatment focuses on other strategies, such as:
- Stroke prevention — managing blood pressure, cholesterol, and blood sugar
- Physical therapy — balance training and learning to use mobility aids like canes or walkers
- Regular aerobic exercise — to reduce the risk of further strokes
- Lifestyle changes — quitting smoking and eating a diet lower in salt and saturated fat
