I’m sorry you’re dealing with this. Severe nighttime leg and foot pain can occur with Parkinson’s, but it has several possible causes: muscle rigidity or cramping, painful dystonia—often seen as curled toes or a foot twisting inward when levodopa wears off—restless legs, neuropathy, medication effects, or an unrelated circulation or orthopedic problem.

Notice whether the pain is burning, aching, cramping, or accompanied by toe curling, and whether it begins before your next Parkinson’s dose. Keep a short record of the timing, symptoms, and medications, then discuss it promptly with your neurologist. Medication timing sometimes needs adjustment, but don’t change Parkinson’s medicine on your own. The doctor may also consider checking iron/ferritin, B12, folate, blood sugar, kidney function, electrolytes, and circulation.

Meanwhile, gentle calf and foot stretching, massage, a warm bath or heating pad, brief walking, supportive footwear, and adequate hydration may help, if medically safe for you. Seek urgent care for sudden severe pain, one-sided swelling or redness, a cold, pale, or blue foot, new weakness, chest pain, or shortness of breath.

I have read about the leg pains and have friends that suffer leg pains. My opinion would be to talk to your doctor about muscle relaxers, it has helped others I know. Also, if you are exercising a lot maybe less intensity. Just my opinion ask your doctor.
