A few thoughts: Are your bills being paid on time? Are you able to keep to a budget? Are you taking your pills on schedule? Are you able to make medical appointments for yourself? Is your home being maintained? Are you getting social interaction? Are you able to plan for, shop, and cook meals? If not, get help.
